You can begin the renewal process within 60 days of the expiration date on your
current card. The PAMMJ team recommends renewing your card at least 3 weeks in
advance to ensure your new patient card is delivered before your current card
expires. It can take up to 7-10 business days for your new card to arrive after
the appointment with the doctor.
New Patient: $49.99
Renewing Patient: $49.99
After your appointment with the doctor, you must log in to your patient profile on the Dept. of Health website and pay the $50 annual fee to the state. Individuals who qualify for the Medical Marijuana Assistance Program (MMAP) are exempt from this fee.

Qualifying conditions include:
- Amyotrophic lateral sclerosis (ALS)
- Anxiety disorders
- Autism spectrum disorders
- Cancer, including remission therapy
- Chronic Hepatitis C
- Crohn’s disease
- Damage to the nervous tissue of the central nervous system (with objective neurological indication of intractable spasticity)
- Dyskinetic and spastic movement disorders
- Epilepsy
- Glaucoma
- HIV/AIDS
- Huntington’s disease
- Inflammatory Bowel Disease
- Intractable (hard-to-control) seizures
- Multiple sclerosis (MS)
- Neurodegenerative diseases
- Neuropathies
- Opioid use disorder (when traditional treatments are ineffective or not suitable)
- Parkinson’s disease
- Post-traumatic stress disorder (PTSD)
- Severe chronic or intractable pain
- Sickle cell anemia
- Terminal illness
- Tourette syndrome
